About The Position
The Care Team Nurse provides coordinated, patient-centered nursing care to support positive health outcomes. This role collaborates with physicians, providers, patients, families, and community resources to assess needs, develop care plans, monitor progress, and ensure continuity of care across healthcare settings.
Requirements
- Current Registered Nurse (RN) license in Indiana or appropriate multi-state licensure
- Minimum of 1-2 years of clinical nursing experience.
Nice To Haves
- Bachelor's degree in Nursing (BSN).
- Certification in Care Management or Case Management (CCM) preferred.
Responsibilities
- Conduct patient assessments and identify healthcare needs.
- Develop, implement, and evaluate individualized care plans.
- Coordinate care among providers, specialists, hospitals, and community resources.
- Educate patients and families on treatment plans, medications, and preventive care.
- Monitor patient progress and follow up on care goals.
- Assist with chronic disease management and health promotion initiatives.
- Review laboratory results, diagnostic tests, and provider recommendations.
- Document patient interactions and care activities accurately within the electronic health record (EHR).
- Respond to patient questions and concerns via phone, electronic communication, or in-person visits.
- Ensure compliance with nursing standards, organizational policies, and regulatory requirements.
- Participate in quality improvement and patient safety initiatives.
