The Care Team Manager is responsible for operating assigned homes in a manner that ensures company goals are met, including meeting or exceeding budget and clinical effectiveness. This role supports and supervises Direct Support Professionals (DSPs), ensuring they have the necessary training and skills for high-quality care. The manager also maintains the assigned homes, ensuring all systems, facilities, and furnishings are in good working order, and assigns maintenance duties to staff when possible, or contacts the appropriate department for facility issues. Additionally, the Care Team Manager assists with contracting for approved services and evaluates contractor performance. In terms of clinical and individual care, this role collaborates with supervisors and the clinical team to ensure proper clinical actions for the health, safety, and stability of individuals. They also oversee individual activities and programs, identifying improvements to enrich social, emotional, mental, and physical needs, and increase home involvement and independent living skills. Regular communication with internal and external clinical, medical, and regulatory agencies is required. The manager must attend individual functions as scheduled and ensure DSPs do the same, maintaining regular individual interaction per person-centered plans.
