About The Position

The Care Specialist provides person-centered assistance to clients as they develop life skills and personal goals. The role coordinates care with a multidisciplinary team, helps clients connect with community resources, and provides crisis intervention and prevention services to clients and their families. The Care Specialist supports clients in individual and group settings and maintains required clinical documentation in accordance with organizational guidelines and compliance standards.

Requirements

Associate's degree required; two years of experience or two years of college coursework in a related field may be substituted for the degree requirement.
Effective written, verbal, in-person, and virtual communication skills.
Customer service skills.
Time management skills.
Analytical skills.
Knowledge of community resources.

Nice To Haves

Two years of professional experience working directly with youth and families in child welfare, a school, childcare, criminal justice, behavioral health, or a related setting.

Responsibilities

Coordinate client care with a multidisciplinary team and help develop and maintain clinical records, including treatment plans, progress notes, reports, and correspondence with external agents.
Teach and train clients in activities of daily living in group and individual settings, while meeting clinical documentation requirements and following organizational guidelines or protocols.
Connect clients with appropriate community support systems.
Maintain regular communication with schools, OFC’s, and other family service providers involved with clients and their families.
Provide telephone and on-site crisis intervention, prevention, and follow-up for clients and their families.
