Requirements

  • High school graduate or equivalent required.
  • 3 years’ experience in health care office, preferably in hospice and/or home health care.
  • Proficient computer skills required
  • Demonstrates excellence in customer service and time management
  • Knowledge of corporate business management, governmental regulations, CHAP standards, and private payer practices.
  • Demonstrates good communications, negotiation, and public relations skills.
  • Demonstrates autonomy, organization, assertiveness, flexibility, and cooperation in performing job responsibilities.
  • Flexible schedule availability (based on business need)

Responsibilities

  • Utilize available resources to identify patients at high risk for rehospitalization and mortality and provide an extra layer of support.
  • Provide education and resources to patients, clinicians and community partners on available care options.
  • Support patients as the navigate the healthcare continuum.
  • Provide personalized advice based on the patient’s unique goals and care needs.
  • Provide guidance to patients during challenging situations and decision-making processes.
  • Assist with advance care planning tailored to a patient's wishes and values.
  • Educate and guide patients regarding healthcare systems, including home health, hospice, palliative care, hospitalizations, and skilled nursing or rehabilitation placement.
  • Maintain a strong sense of responsibility in responding to patients and clinicians, ensuring timely and effective support is provided as needed.
  • Ability to maintain careful records and document all patient/family interactions.
