About The Position

The Care Plan Coordinator oversees and coordinates the development and completion of the participant plan of care in accordance with current Federal and State regulations, and guidelines that govern the participant assessment, to include the policy and procedures of InnovAge. This role is responsible for tracking completed care plans for compliance and maintaining the Care Plan schedule spreadsheet. The coordinator facilitates Care Plan discussions for the Interdisciplinary Team, ensuring the team engages in conversations and documentation needed to improve or maintain the participant’s functional abilities at the highest practicable level. This includes auditing EMR, having team discussions in Morning Meeting, Care Planning, and mini teams to ensure documentation, diagnosis, etc. match up and have corresponding coverage in the care plan. The position oversees the assessment and care plan process for timeliness and participates in various committees of the center for quality Assessment and Assurance as required. The Care Plan Coordinator works collaboratively with all new InnovAge teams to encourage timely, accurate, and quality assessments and care plans, specifically scheduling, monitoring for timeliness, compliance, etc. Additionally, this role develops a plan to provide Care Plan training for all new hires and yearly training for all other disciplines. The coordinator must understand, comply with, and promote all rules and regulations regarding participants' rights, promote positive relationships with participants, families, visitors, and regulators, and present and communicate with a professional image at all times. Adherence to InnovAge Code of Compliance, conduct and Business Ethics policy, including documentation and reporting responsibilities, is mandatory. The role also involves attending and participating in educational activities, in-service training, staff meetings, and Quality Improvement activities as assigned.

Requirements

  • Active LPN license to practice in the State OR Graduate of an accredited Medical Assistant program OR current state issued Certified Nursing Assistant CNA license OR a healthcare related Bachelors degree.
  • Current CPR certification or the ability to obtain CPR certification.
  • 3+ years of experience in a health care setting.
  • Knowledgeable of nursing and medical practices and procedures, laws, regulations and guidelines that pertain to PACE.
  • Excellent computer technology including e-mail use.
  • Proven written and oral communication skills with strong analytical and decision making skills.
  • Must be capable of maintaining regular attendance.

Responsibilities

  • Track completed care plans for compliance and maintain the Care Plan schedule spreadsheet.
  • Facilitate Care Plan discussion for the Interdisciplinary Team by engaging the team in conversation and documentation needed to improve or maintain the participant’s functional abilities at the highest practicable level (including auditing EMR, having team discussions in Morning Meeting, Care Planning, mini teams, etc. to ensure documentation, diagnosis, etc. match up and have corresponding coverage in the care plan).
  • Oversee the assessment and care plan process for timeliness.
  • Participate via attending various committees of the center for quality Assessment and Assurance as required, as appointed by the Clinic Service Manager or Center Director.
  • Work collaboratively with all new InnovAge teams to encourage timely, accurate and quality assessments and care plans – This position is responsible for timeliness of care plan audit documentation and completion – specifically scheduling, monitoring for timeliness, compliance, etc.
  • Develop a plan to provide Care Plan training for all new hires at their respective center as well yearly training for all other disciplines.
  • Understand, comply with and promote all rules and regulations regarding participants rights, promote positive relationships with participants, families, visitors and regulators, and present and communicate with a professional image at all times.
