About The Position

Join a values-driven local team helping older Victorians, people living with disability and TAC-funded participants live well at home. Just Better Care Gippsland is a locally owned franchise of the national Just Better Care network. Based in Rosedale, we deliver in-home support across Gippsland under Support at Home, the NDIS and the TAC. We're known for quality care, strong clinical governance and person-centred, consumer-directed support. As a Care Partner, you'll be the trusted point of contact for a caseload of participants, working with them, their families and the wider care team to develop, deliver and review individualised care plans. You'll work mainly with Support at Home participants, with opportunities across NDIS and TAC as your experience grows. You'll be backed by an experienced Clinical Team Leader, fellow Care Partners and an office-based team.

Requirements

  • Experience in case management, care coordination or a similar role
  • Strong understanding of person-centred, consumer-directed care
  • Excellent written, verbal and interpersonal skills, and experience working in a multi-disciplinary team
  • Digital confidence with participant management systems
  • Strong organisation and time management to balance a varied caseload
  • Openness to constructive feedback and commitment to continuous improvement
  • Current First Aid Certificate (within the last 3 years) and CPR Certificate (within the last 12 months)
  • Current, clear NDIS Worker Screening Check
  • Australian Citizen or Permanent Resident
  • Current Australian driver's licence

Nice To Haves

  • Qualification in Individual Support (Ageing), Aged Care, Disability, Community Services or Nursing
  • Experience with Support at Home, NDIS and/or TAC guidelines
  • Experience with AlayaCare or a similar system

Responsibilities

  • Complete non-clinical care planning that captures each participant's needs, goals, preferences and existing supports
  • Develop and review care plans in partnership with participants and their representatives, in line with program guidelines
  • Coordinate care teams and external providers so services are safe, on schedule and within scope
  • Monitor changing needs and emerging risks, escalating clinical concerns to the Clinical Team Leader
  • Keep accurate records and track participant budgets in AlayaCare
  • Build relationships with GPs, allied health, hospitals and community providers across Gippsland
  • Help participants understand their rights, navigate the system and give feedback
  • Contribute to continuous improvement and uphold quality, WHS and compliance standards
