About The Position

The Care Partner (CP) is a support care provider who functions under the direction of the Registered Nurse. The Care Partner's primary focus is to promote a positive patient experience by partnering with the nurse, physician, healthcare team, and other healthcare providers to deliver excellence in patient experience. They assist with daily living activities and comfort measures. The Care Partner assists other healthcare providers with safe, age-appropriate, and culturally aware care by performing basic patient care functions such as ADL's, ambulation, and vital signs. Depending on unit needs and approved advanced technical training, the Care Partner may perform skills such as phlebotomy, simple treatments, dressing changes, oral suctioning, and discontinuing Foley catheters. Clerical functions, including data retrieval, maintaining unit supplies, and transporting supplies/patient medications, are also required. The role demands the ability to work in a fast-paced environment requiring multi-tasking and effective communication with the Registered Nurse and ancillary team. The Care Partner must perform under stressful conditions with a compassionate, respectful, and service-oriented mindset, be able to take direction from various care providers, and possess strong organizational and communication skills.

Requirements

Must be 16 years of age or older.
BLS - Basic Life Support or ARC-BLS - Amer Red Cross Basic Life Support or BLS-I - Basic Life Support - Instructor certification required upon hire.
Ability to follow detailed directions.
Strong interpersonal skills.
Clear verbal and written communication, specifically ability to document.
Ability to work individually and as part of a team.
Wellstar minimum age of hire is 16.

Nice To Haves

Rising Senior in Good Standing Other / Related Field or High School Diploma / GED General-Preferred
Graduation from a State approved program for Certified Nursing Assistants or Patient Care Tech Program Upon Hire Preferred

Responsibilities

Provide excellence in patient experience by partnering with the healthcare team, patient, and family.
Support an environment of healing or wellness for patients, addressing patient needs and following the plan of care as assigned or instructed by a licensed professional.
Engage the patient and family in compliance with the treatment plan while carrying out role responsibilities.
Effectively contribute to the plan of care or treatment based on assigned tasks and patient/family input in collaboration with the healthcare team, focusing on the care continuum.
Consistently implement all safety interventions to minimize risk in achievement of intended outcomes.
Protect the rights and dignity of patients.
Maintain confidentiality adhering to HIPAA regulations and recommended privacy procedures.
Participate in hourly patient rounding.
Answer telephone and call lights promptly and professionally, notifying the appropriate individual of the need for follow-up.
Resolve patient concerns or escalate to the charge nurse if unable to resolve.
Proactively offer assistance and comfort measures to patients and families.
Present self in a positive and professional manner, showing sensitivity to the concerns and needs of others.
Provide patient care under the direction of the RN, including hygiene, nutrition, elimination, and physical comfort measures.
Perform advanced skills based on unit-specific requirements (e.g., phlebotomy, EKG, removal of Foley catheters and IVs).
Assist the RN and other members of the healthcare team with care delivery as assigned.
Assist in direct patient admission.
Monitor data entry for key patient safety information.
Participate in charge reconciliation.
Perform audits as requested.
Support initiatives of the healthcare team.
Participate in unit performance improvement initiatives.
Support professional nursing practice through shared leadership, evidenced-based practice, and ethical accountability.
Transport unit supplies, patient care supplies, medications, and equipment as assigned.
Practice accountability.
Use effective written and oral communication skills with patients, staff, physicians, family, and visitors (e.g., AIDET).
Use safety communication techniques (e.g., ARRC).
Communicate completion of assignment/duties to the RN or Charge Nurse.
Document completion of patient care in the EMR.
Communicate patient care concerns/problems/emergencies to the RN or Charge Nurse in a timely manner.
Maintain equipment (e.g., blood pressure machines, glucometers) and report malfunctioning devices.
Communicate with patients and update whiteboards with information.
Maintain and act as a unit expert regarding patient care documentation tools, including during downtime.
Provide a safe environment for patient care.
Consistently use proper hand hygiene.
Keep patient rooms and unit areas clean and free of clutter.
Follow appropriate PPE/Infection control policies.
Reinforce the importance of universal precautions with patients and their families.
Utilize proper body mechanics, patient safe handling devices, and assistive devices.
Perform environment of care rounds.
Comply with all Wellstar Health System policies, standards of work, and code of conduct.
Perform other duties as assigned.
