About The Position
The Care Partner plays a vital role in supporting the health and well-being of patients by providing compassionate, patient-centered care in a variety of healthcare settings. This position focuses on assisting nursing staff and other healthcare professionals with daily patient care activities, ensuring comfort, safety, and dignity for each individual. The Care Partner acts as a key liaison between patients and the clinical team, observing and reporting changes in patient conditions promptly. Through effective communication and teamwork, the Care Partner contributes to a positive healing environment and enhances the overall patient experience. Requirements
- High school diploma or equivalent.
- Basic understanding of patient care principles and safety protocols.
- Ability to communicate effectively with patients, families, and healthcare team members.
- Physical ability to assist patients with mobility and perform tasks that require standing, lifting, and bending.
- Strong communication skills daily to interact compassionately with patients and effectively relay information to the healthcare team.
- Attention to detail is critical when monitoring patient conditions and documenting observations accurately to ensure timely interventions.
- Physical stamina and dexterity are essential for assisting patients with mobility and performing various care tasks safely.
- Problem-solving skills help the Care Partner respond appropriately to patient needs and unexpected situations.
- Teamwork and adaptability enable the Care Partner to collaborate efficiently within a dynamic healthcare environment, contributing to high-quality patient care.
Nice To Haves
- Previous experience in a healthcare or caregiving role preferred but not required.
- Certified Nursing Assistant (CNA) certification or equivalent.
- Experience working in a hospital, long-term care, or rehabilitation setting.
- Training in CPR and basic first aid.
- Familiarity with electronic health record (EHR) systems.
- Strong interpersonal skills and cultural competency to work with diverse patient populations.
Responsibilities
- Assist patients with activities of daily living such as bathing, dressing, grooming, and mobility to promote independence and comfort.
- Monitor and document patient vital signs, behaviors, and overall condition, promptly reporting any changes to nursing staff.
- Support nursing staff with clinical tasks including specimen collection, patient transport, and maintaining a clean and safe environment.
- Provide emotional support and companionship to patients, helping to alleviate anxiety and promote mental well-being.
- Collaborate with interdisciplinary healthcare teams to ensure coordinated and comprehensive patient care.
