About The Position

The Care Partner IV (CNA License Required) is a part-time position within the Float Pool at Community Hospital, working variable shifts during the day. This role is crucial for ensuring optimal patient care across various inpatient units by providing direct assistance and support to patients. The position requires flexibility, adaptability, and a commitment to upholding the hospital's standards of care and equal employment opportunity.

Requirements

  • Current Colorado Certified Nursing Assistant certification.
  • Current Basic Life Support (BLS).
  • A minimum of four (4) years of experience.
  • High school diploma or equivalent.
  • Completion of a state-approved CNA training program.
  • Competent to float to all inpatient units, demonstrating flexibility and adaptability to diverse patient populations and care protocols.
  • Must relocate to Grand Junction, CO 81505 before starting work.

Responsibilities

  • Responding to signal lights, bells, intercom system, and bed/tab alarms to determine patients' needs.
  • Providing care and assistance to patients regarding personal hygiene, grooming, activity, comfort, rest, nourishment, and other health needs.
  • Ensuring meals are ordered within dietary restrictions, collecting food trays, feeding patients requiring assistance, and documenting nutritional and supplemental intake.
  • Transporting patients or assisting them to ambulate as per physician orders.
  • Turning and repositioning patients every two hours to prevent skin breakdown and pressure ulcers.
  • Applying compresses, ice bags, ted hose, and SCDs as indicated.
  • Completing basic patient data gathering, including blood pressure, pulse (O2 saturation), respiration rates, height and weight, and food and fluid intake/output.
  • Reporting abnormal findings immediately to the primary nurse or responsible nurse.
  • Obtaining point-of-care blood glucose and reporting results to the RN.
  • Changing bed linens and directing visitors.
  • Organizing and stocking patient rooms as needed, ensuring rooms are tidy and free of environmental hazards.
  • Preparing charts for admissions, discharges, and transfers.
  • Floating to various units as needed to ensure optimal patient care.
