About The Position

Unison Health is seeking a full-time Care Navigator to be placed within our Perrysburg facility. The Navigator will assist clients with securing and maintaining housing within the community, monitoring their placement for stability. This role involves performing case management duties, helping clients overcome barriers to obtaining and maintaining resources, and ensuring confidentiality in accordance with federal guidelines and agency policy. The position requires participation in team meetings, supervision, in-service training, and conferences.

Requirements

  • High school diploma required.
  • Minimum of a CDCA license required.
  • Must be able to work a flexible schedule that may occasionally include evenings and weekends.

Nice To Haves

  • Four-year degree in social work, psychology, or related field preferred.
  • LCDC II/III or LICDC preferred.

Responsibilities

  • Manage an assigned caseload and provide ongoing support, advocacy, and resource coordination for clients.
  • Assist individuals and families in securing and maintaining safe, stable housing within the community.
  • Connect clients to community resources, benefits, and supportive services that promote long-term stability and self-sufficiency.
  • Help clients identify and overcome barriers to obtaining and maintaining housing and other essential resources.
  • Monitor client progress through regular follow-up, service coordination, and housing stability checks.
  • Maintain accurate, timely, and compliant documentation of client interactions and services provided.
  • Ensure confidentiality and proper handling of protected information in accordance with federal regulations and agency policies.
  • Collaborate with internal teams, external providers, and community partners to support positive client outcomes.
  • Participate in team meetings, supervision, training, and professional development opportunities.
  • Make a meaningful impact on the lives of individuals and families by promoting housing stability and overall well-being.
  • Serve in a community-focused role that combines case management, advocacy, housing support, and resource navigation.
  • Work within a collaborative and supportive team environment with opportunities for ongoing training and professional growth.
  • Help clients achieve greater independence, stability, and long-term success through individualized support and service coordination.
