About The Position

The Care Navigator reports to the Director of Emergency Services and serves as an integral team member of a statewide initiative to improve access to Medication for Opiod Use Disorder (MOUD). This role conducts initial brief assessments, introduce patients to MOUD programs and services, expedites appointments at Medication for Addiction Treatment (MAT) capable clinics, serves as the primary coach for their clients , and maintains ongoing contact with the California Bridge Program panel. The Care Navigator assists with access to other services such as financial counseling, primary care, mental health services, social services and residential treatment facilities to link patients from the hospital to the community. The tradition of caring that culminated in the establishment of Corona Regional Medical Center® began in 1965, with the founding of Corona Community Hospital. Since that time the hospital has changed to meet the needs of the rapidly growing community. Corona Community Hospital merged with Circle City Medical Center in 1992 and the resulting entity became Corona Regional Medical Center. The merged hospitals are now a 238-bed community hospital network comprised of a 160-bed acute care hospital and a 78-bed rehabilitation campus. Requirements

  • Associate’s degree from an accredited College or University in health care related field required.
  • Two (2) years of experience in a drug treatment environment required.

Nice To Haves

  • Bachelor's degree from an accredited College or University in related field of substance abuse, crisis management or counseling preferred.
  • Recent in-patient drug treatment experience preferred.
  • Recent in-patient hospital experience preferred.
