Care Navigation Concierge, Day Shift, Outpatient Care Navigation
About The Position
White Oak Medical Center seeks to hire an experienced Care Navigator Concierge for our Outpatient Care Navigation area who will embrace our mission to extend God’s care through the ministry of physical, mental, and spiritual healing. As a Care Navigator Concierge, you will schedule appointments for patients and families in accordance with discharge instructions. You will maintain awareness of local physician practices and the manner in which to schedule hospital follow-up appointments. You will notify your manager when experiencing challenges to booking hospital follow-up appointments and notify other members of the Patient Navigation team when patients require additional support to see a physician post discharge. You will also complete required documentation in a timely and accurate manner.
Requirements
- High School Diploma
- Strong customer services skills
- 1-2 years’ experience working in a medical setting
- Active American Heart Association Basic Life Support (BLS) certification required
Responsibilities
