This position requires demonstrated experience and/or training in the assessment and treatment of adolescents and adults seeking psychiatric and/or chemical dependency treatment. Working under the supervision of the Manager/Director, the role collaborates with the healthcare team to coordinate patient care across the continuum. The Care Manager promotes effective utilization and monitoring of healthcare resources and leads the interdisciplinary team to achieve optimal quality, clinical, and resource outcomes. Responsibilities include performing and completing all medical record documentation and other written assignments (CM Screening Form) as outlined in department/hospital policy. The Care Manager also serves as the court services liaison and is an integral part of the multidisciplinary treatment team. This role is responsible for discharge planning to ensure aftercare plans align with patient goals and are in accordance with Riverside Health System's standards of excellence, facility goals, and strategic plans. The position utilizes the ACMA scope of Services and Standards of Practice as a foundation for Care Management and completes annual competencies to ensure evidence-based care. Non-Registered Nurse and Non-Social Work staff are required to convey discharge plans with their supervisor and the entire interdisciplinary team.
