About The Position

The Autism Society of North Carolina (ASNC) is growing its Tailored Care Management team. ASNC is a high-quality Care Management agency with one of the highest engagement rates in the state. As a niche program, it offers Care Managers and families a truly individualized experience focused on quality. This is a hybrid position that requires travel around the Triangle Area (Wake, Durham, Orange Counties), NC.

Requirements

• Bachelor’s or master’s in human services (Social Work, Psychology, etc.)
• 2+ years' experience with Autism and/or Intellectual /Developmental Disabilities
• 2+ years’ experience providing care management, case management, or coordination to individuals with IDD/ASD
• Based in or near the Triangle area and willing to travel locally
• Strong communication, organization, and problem-solving skills
• Self-starter who thrives in a collaborative environment

Responsibilities

• Provide whole-person care management for children with autism or other developmental disabilities
• Collaborate with care teams, service providers, and families
• Work with families to identify needs and provide resources and support to meet those needs
• Submit referrals, manage authorizations, and monitor service delivery
