About The Position

Under general supervision, Care Managers assist in providing rehabilitative, restorative treatment services, crisis intervention, therapeutic intervention, assessment evaluation, and moderately complex, professional-level case management. This includes visiting clients in residential or other settings, making referrals, counseling clients on various issues, and providing one-on-one behavioral support.

Requirements

  • Care Manager I: Less than two (2) years of FTE experience in a mental health-related field providing direct mental health service.
  • Care Manager II: At least two (2) years but less than four (4) years of FTE experience in a mental health-related field providing direct mental health services.
  • Care Manager III: At least four (4) years of full-time/equivalent (FTE) experience in a mental health-related field providing direct mental health services.
  • Up to two (2) years of the four (4) years of experience for Care Manager III can be substituted by years of education in a mental health-related field.
  • Minimum requirement of two (2) years of actual work experience for Care Manager III.
  • Education requirement for Care Manager III: Minimum of two (2) years of education (60 semester or 90 quarter units) with a minimum of 12 semesters (18 quarter) units in a mental health-related subject area such as child development, social work, human behavior, rehabilitation, psychology, or alcohol and drug counseling.
  • Must possess a valid CA Driver’s License, current auto insurance, and a clean DMV printout (no more than 3 points).
  • Must pass a clear background check with CACI, DOJ & FBI.
  • CPR and First Aid Certified.
  • TB Exam and Physical at the time of employment.

Responsibilities

  • Demonstrates activities that address the purpose of the Rehabilitative group and individual client plan.
  • Follows the State Mental Health and County Mental Health guidelines for any service being provided.
  • Converses with clients to reinforce positive behaviors and to promote social interaction.
  • Serves meals and eats with clients to act as a role model as it may apply to rehabilitative or behavior plans.
  • Menu plans and grocery shops with clients.
  • Plans and executes regular scheduled activities or outings with the clients for purposes of skill-building.
  • Provide medication support to clients as needed and completes required documentation.
