About The Position

The Care Manager provides direct, relationship-centered support to youth and families enrolled in the C.A.R.E. Early Intervention Program. This role focuses on weekly engagement, individualized care planning, advocacy, and coordination with families, schools, and community partners to help address barriers before they escalate. As the primary point of contact, the Care Manager supports youth stability, personal growth, and positive long-term outcomes through consistent in-person meetings, ongoing assessment, and connection to supportive services.

Requirements

  • Bachelor’s degree in Social Work, Psychology, Education, Human Services, Counseling, or related field
  • Minimum one year experience working with youth and families
  • Strong written and verbal communication skills
  • Valid driver’s license and reliable transportation required
  • Maintain up to 20 active cases
  • Complete weekly face-to-face, in person contact with youth
  • Submit case notes within 24-48 hours of service to child
  • Participate in bi-monthly team meetings and trainings
  • Ability to work flexible hours including evenings and weekends
  • Must provide proof of reliable transportation
  • Must complete required background clearances

Nice To Haves

  • Master’s degree in Social Work, Counseling, or related field
  • Training in trauma-informed care, crisis intervention, or restorative practices

Responsibilities

  • Manage a caseload of up to 20 youth per assigned contract
  • Conduct regular face-to-face, in person contact with youth and families
  • Build strong, trust-based relationships with youth and caregivers
  • Provide crisis intervention and support as needed
  • Assist with ongoing assessments of youth and families
  • Gather collateral information from schools, agencies, and providers
  • Develop and coordinate individualized treatment plans
  • Complete documentation accurately and on time
  • Advocate for youth and families across systems
  • Collaborate with community advocacy groups and Managed Care providers
  • Empower families to actively participate in managing care and services
  • Coordinate referrals and access to services
  • Collaborate with treatment teams, schools, and community partners
  • Monitor the effectiveness and quality of services provided
  • Maintain accurate and timely case notes and progress reports
  • Track participation, engagement, and outcomes
