About The Position

Crossroads Treatment Centers is seeking a dedicated Care Manager to join their team in Erie, PA. With clinics across the U.S., they serve over 26,500 patients in outpatient, office-based settings. This role involves connecting with patients monthly to address their needs, providing resources, and maintaining relationships with community partners. The Care Manager will work closely with a dedicated team to ensure exceptional patient care and participate in community events to promote Crossroads' mission and treatment approach.

Requirements

At minimum, High School Diploma or GED required.
Knowledge of compliant documentation practices

Nice To Haves

Experience in a fast-paced environment, servicing many clients, is preferred.
Experience in healthcare, addiction medicine, community health, care coordination preferred.
Experience with building individualized care plans is a plus.
Lived Addiction experience is a plus.

Responsibilities
