About The Position

The Part-Time Care Manager – Social Isolation Grant provides individualized care management services to older adults participating in a grant-funded initiative designed to reduce social isolation and improve overall well-being. This position conducts assessments, develops personalized care plans, connects participants to community resources, and provides ongoing support through home visits and telephone outreach. The Care Manager also ensures compliance with grant requirements through accurate documentation, data collection, and outcome reporting. Older adults experiencing social isolation often face increased health risks and reduced quality of life. This role plays a critical part in helping participants remain connected, access essential resources, and maintain their independence while supporting Bergen Family Center's commitment to improving the health and well-being of the communities we serve.

Requirements

  • Valid New Jersey driver's license with reliable transportation required.
  • Ability to successfully complete all required background checks.
  • Bilingual in English and Spanish required.
  • Excellent verbal and written communication skills.

  • Experience using electronic case management or client database systems.
  • Ability to accurately collect, track, and report program data.
  • Comfortable using virtual communication platforms and basic office technology.

Nice To Haves

  • Associate's or Bachelor's degree in Social Work, Human Services, Psychology, Gerontology, or a related field preferred.
  • Minimum of two years of experience working with older adults preferred.
  • Case management experience preferred.
  • Experience with community-based or grant-funded programs is a plus.

Responsibilities

  • Conduct comprehensive in-home assessments to identify participant needs, strengths, risks, and barriers to engagement.
  • Develop, implement, monitor, and update individualized care plans based on participant goals.
  • Conduct regular in-home visits and telephone outreach to reduce social isolation and promote participant well-being.
  • Monitor participant progress and adjust care plans as needed.
  • Connect participants with community resources, healthcare providers, support services, and benefit programs.
  • Identify unmet needs and coordinate appropriate referrals to enhance participant independence and quality of life.
  • Maintain accurate, timely, and confidential participant records in accordance with agency policies and grant requirements.
  • Complete required assessments, surveys, case notes, and outcome documentation.
  • Collect and report participant data to support grant compliance, program evaluation, and performance measures.
  • Ensure all documentation meets funding source and organizational standards.
  • Build trusting relationships with participants through compassionate, person-centered care.
  • Communicate regularly with family members, caregivers, and other support systems, as appropriate.
  • Educate participants and families about available community, state, and federal resources and services.
  • Advocate for participant needs while promoting dignity, independence, and informed decision-making.
  • Collaborate with internal staff, healthcare providers, community organizations, and social service agencies to coordinate participant care.
  • Participate in team meetings, case reviews, and professional development opportunities.
  • Maintain effective communication with supervisors regarding participant needs, service outcomes, and program updates.
  • Perform other duties as assigned to support program and organizational goals.
