Program Overview To keep families together by providing safe, permanent housing, and ongoing accessibility to evidence-based trauma informed support services to Child Protection and Permanency families throughout our region.
Requirements
- Bachelor's level practitioner or high school graduate with 3 or more years of experience providing direct Case to youth in a behavioral health agency or institutional setting.
- Must have and maintain a valid driver's license with an acceptable driving record
- Must be able to exercise independent thinking and good judgment under all circumstances.
- Must be able to analyze work, set goals, develop plans and utilize time effectively and efficiently.

Responsibilities
- Provide support services and case management to families with a history of homelessness or housing instability.
- Assist parents to make a successful transition to self-sufficient living and improved individual and family stability.
- Provide coaching and guidance on life skills such as home management, nutrition, budgeting, parenting, education/vocation training, advocacy skills
- Co-facilitate groups for parents, children and families.
- Link families to both formal and informal community resources.
- Work collaboratively with your KFT peers, DCP&P and community service providers.
- Complete documentation consistent with program and agency requirements.
