About The Position

Community Care is hiring a full-time Case Manager/Care Manager for its Family Care Program in Milwaukee, Wisconsin. The position supports older adults and adults with physical and intellectual disabilities in living as independently as possible in the community. The role involves team collaboration and community nursing case management. The schedule is Monday through Friday, 8:00 a.m. to 4:30 p.m., and the position requires regular local travel using a personal vehicle. Community Care notes that it is easing caseloads to support employee work-life balance.

It serves more than 13,000 Wisconsin residents and has local offices supporting the 23 counties it serves. Its teams work with members and community partners to develop care plans addressing health and social needs and to support compassionate, personalized care.

Requirements

• Bachelor’s degree in Human Services or a closely related field.
• At least 3 years of recent experience working with older adults or people with physical or intellectual disabilities.
• Eligibility for long-term care functional screener certification.
• Valid driver’s license and proof of required insurance coverage for use of a personal vehicle.

Nice To Haves

• Current long-term care functional screener certification is preferred.
• Three years of community nursing work is preferred.

Responsibilities

• Manage a caseload of Family Care Program members.
• Complete comprehensive clinical assessments.
• Collaborate to develop member-centered care plans.
• Provide ongoing care coordination, member education, quality assurance, and monitoring of services.
• Travel regularly within the local area using a personal vehicle.
