About The Position

Provides intensive, community-based case management services to individuals enrolled in Assertive Community Treatment (ACT), with a primary focus on Assisted Outpatient Treatment (AOT) clients. Responsibilities include conducting outreach, coordinating care, developing and implementing person-centered treatment plans, monitoring compliance with court-ordered treatment requirements, connecting clients to community resources, advocating for client needs, and collaborating with multidisciplinary team members, families, and community partners. Supports clients in achieving recovery goals, increasing stability, reducing hospitalizations, and maintaining successful community living through ongoing assessment, crisis intervention, and rehabilitative services.

Requirements

  • Master’s Degree in a human services related field OR Bachelor’s Degree in a human services related field and 1 year relevant experience working with the target population OR High School Diploma or GED with a minimum of 3 years of experience working with the target population OR Certified Peer Specialist (CPS) or Certified Family Specialist (CFS)
  • Valid drivers’ license and meets the driving privileges guidelines established in HR Policy 12.6 “Vehicle Safety and Client Transportation.”
  • Ability to manage time to meet all deadlines.
  • Ability to recognize and identify symptoms of mental illness and suicide and risk potential and respond appropriately.
  • Good verbal and written skills.

Nice To Haves

  • Bilingual (English/Spanish) is preferred but not required.

Responsibilities

  • Deliver community-based services and outreach to AOT clients.
  • Coordinate medical, behavioral health, housing, and social service needs.
  • Monitor treatment engagement and compliance with AOT requirements.
  • Maintain timely and accurate clinical documentation.
  • Participate in ACT team meetings and interdisciplinary treatment planning.
  • Provide crisis intervention and support recovery-oriented services.
  • Foster client independence, skill development, and community integration.
