About The Position

RIGHT ACCORD Home Health Care is seeking a Care Manager to bridge the gap between clients, their families, and caregivers. This role involves being hands-on in client homes and communicating with families and caregivers to ensure safe, consistent, and attentive care. The Care Manager will oversee the quality of care for clients across Sarasota County, attend new client's first shift, supervise and coach caregivers, monitor client well-being, build relationships with clients and families, resolve concerns, and partner with scheduling and administration to ensure seamless coverage and uphold safety standards.

Requirements

Active LPN license required
Active RN license strongly preferred
Genuine warmth paired with strong follow-through and accountability
Comfortable being the point person families call when something needs attention
Strong organizational skills and the ability to manage multiple clients and caregivers at once
A calm, clear communicator, in person, on the phone, and in writing
Reliable transportation and willingness to travel between client homes in Sarasota County
Home care experience preferred

Responsibilities

Oversee the day-to-day quality of care for clients across Sarasota County
Physically attend every new client's first shift alongside the caregiver, ensuring a smooth and confident start
Supervise, support, and coach caregivers in the field
Monitor client wellbeing and communicate changes in condition to the appropriate team members
Build strong relationships with clients and families so they feel heard, supported, and cared for
Step in quickly when a client or family raises a concern, and resolve it before it becomes a bigger issue
Partner closely with scheduling and administration to keep coverage seamless
Help uphold the standards that keep our clients safe, engaged, and well cared for
