About The Position

The Care Manager will be responsible for screening and assessing individuals, coordinating services, and providing direct patient care. This role involves collaborating with various stakeholders, maintaining case records, and educating patients on health issues and care plans. The position requires a strong understanding of nursing standards and practices, proficiency in electronic health systems, and excellent communication and organizational skills.

Requirements

  • A valid, current Registered Nursing License in the state of West Virginia.
  • A valid state-issued driver’s license.
  • Prepares basic correspondence and simple reports in Microsoft Word.
  • Uses Microsoft Excel to create tables and simple displays of information.
  • Creates basic presentations in Microsoft PowerPoint.
  • Demonstrates necessary proficiency with all electronic clinical systems, including EHR and scheduling systems, in use at the Company.
  • Displays basic organizational skills in order to self-prioritize and organize own work.
  • Determines effective responses to situations encountered within established precedent.
  • Able to effectively communicate opinions drawn from conclusions using inference and logic.
  • Exercises tact and diplomacy in the resolution of mild conflicts or disagreements.

Nice To Haves

  • Formal training in nursing, usually in the form of a major in Registered Nursing as part of an associate’s degree, bachelor’s degree, or master’s degree Program.
  • Maintains current knowledge of standards of care and practices, typically acquired through continuing education.

Responsibilities

  • Conducts interviews with individuals (and legal guardians as indicated) to collect information both initially and ongoing.
  • Takes and records vital signs, administers injections, checks, reads, and records blood sugar, draws blood, and collects urine drug screens as ordered by physician/practitioner.
  • Collaborates with individuals, families, physicians, health care providers, and team members.
  • Participates in comprehensive care plan meetings and transitional care planning.
  • Schedules appointments and ensures transportation for individuals served.
  • Prepares and maintains case records within program guidelines.
  • Provides medication reconciliation and education.
  • Educates served regarding a variety of health issues to ensure understanding of care plan and needed action.
  • Encourages individuals to remain in a continuum of care to ensure success.
