The Care Manager provides case management assessment, person centered planning and documentation, referral and linkage, and monitoring/follow-up under direct and indirect supervision. This role ensures metrics for outcomes are met, participates in interdisciplinary treatment planning and consultation, and provides crisis intervention consultation.
