About The Position

The Care Manager is responsible to affect a safe, comfortable and supportive environment for the clients participating in the program. The Care Manager assists clients adjusting to the program routine. The Care Manager documents progress in the medical record and assists the clients with all case management activities.

Requirements

  • Minimum of a high school diploma.
  • At least twelve months’ experience working in the behavioral health field.
  • Knowledge of the Principles and Practices of the addictions and mental health field.
  • Knowledge of Joint Commission Standards.
  • Demonstrates Proficiency in Communication & Written skills.
  • Knowledge of State & Federal Statutes Regarding Patient Confidentiality laws.
  • Knowledge of Drug-Free Workplace Policies.
  • Knowledge of Workplace Violence Policies.
  • Knowledge of addictions and mental health complications.
  • Knowledge of community health resources that support life improvement in physical, emotional, mental and spiritual health, economic security, employment, positive social and family interdependence and positive cultural identity including: Referral procedures and admittance criteria for health care and community providers.
  • Ability to network and collaborate with healthcare and other social and cultural agencies that can provide services to the clients and families.
  • Knowledge of the 12-Step Recovery Program.
  • Knowledge and competency in problem solving, stress management, ethics and team building.
  • Knowledge of alcohol and other drugs that includes: Alcohol and addictive drugs and their physical, emotional, intellectual, and spiritual impact on the individual. Impact of addiction on the family.
  • Ability to determine if a medical emergency exists and to take appropriate action, when necessary.
  • Knowledge of emergency procedures used in case of alcohol and/or drug overdose.
  • Knowledge of the stages of alcohol and other drug withdrawals and ability to take appropriate action at each stage.
  • Knowledge of Federal, State and Municipal laws and/or regulations that regulate the treatment of alcoholism and drug addiction, including: Confidentiality Patient Rights Professional Code of Ethics Discrimination.
  • Knowledge of airborne and blood borne pathogens (TB, HIV, AIDS, HB) and risk reduction methodologies and the use of universal precautions.
  • Flexibility to adapt to schedule changes and assumption of responsibilities not delineated in the job description which are related to work as a member of an addiction treatment team.
  • Current (five-years) of no felonies or substance use convictions.

Nice To Haves

  • Detoxification experience preferred (Detox Counselor Position Only).
  • CPR certification preferred.
  • Bachelor’s Degree from accredited university or equivalent. Experience may substitute for a degree.
  • Florida Requirement: Substance Use: CRSS certification and above (preferred or willing/able to obtain).
  • Maine Requirement: CPR certification preferred.
  • New Hampshire Requirements (if completing independent clinical assessments): Substance Use: A Master Licensed Alcohol and Drug Counselor (MLADC) or a Licensed Alcohol and Drug Counselor (LADC) with an active and unrestricted license.
  • New Mexico Requirements: Substance Use: Licensed Alcohol and Drug Abuse Counselor (LADAC); Licensed Substance Abuse Associate (LSAA); Certified Alcohol and Drug Abuse Counselors (CADC), Certified Peer Support Workers (CPSW).
  • Nebraska Requirements: Substance Use: Active and unrestricted Nebraska Licensed Alcohol and Drug Counselor (LADC) credential.
  • Utah Requirements: Substance Use: Utah Certified Substance Use Disorder Counselor (SUDC) or Associate SUDC (ASUDC) required if providing SUD counseling services.
  • Vermont Requirements: Substance Use: Licensed Alcohol and Drug Counselor, Certified Alcohol and Drug Counselor or Apprentice Addiction Professional.
  • Georgia Requirements: Substance Use: Licensed Professional Counselor (LPC), Licensed Clinical Social Worker (LCSW), Licensed Marriage and Family Therapist (LMFT) or Licensed Psychologist.
  • Arizona Requirements: Substance Use: Licensed Independent Addiction Counselor, Licensed Associate Addiction Counselor, Licensed Addiction Counselor Technician, Licensed Professional Counselor, Licensed Associate Counselor or Licensed Clinical Social Worker.

Responsibilities

  • Work with 3rd parties to connect client to appropriate after care setting, which includes: Obtaining necessary releases of information (ROI) from clients, Provide documentation to outside parties when necessary, for client continued programming (ie. Court attorney, FMLA letters or supporting documentation), Stay in close communication with the clinical team to determine aftercare recommendations, Contacting potential referring agency, Coordinate with client, family and receiving provider for successful transition.
  • Assist with discharge planning and documentation, which includes: Follow up with Clinical Director/Nursing to confirm discharge date and time, Complete required discharge planning paperwork, Coordinate transportation for client’s transition from the facility to the receiving facility if applicable.
  • Facilitate appropriate interventions with clients displaying behavioral or mental instability and clients at risk for leaving Against Medical Advice (AMA): Contact appropriate referents and family members for which agency has release of information. (Informing these parties of clients’ intent and soliciting their assistance either by phone or in person), Make contact with the Clinical Director for additional suggestions or recommendations to assist in redirecting clients, Make contact with the Clinical Director to determine if clients are potentially a harm to themselves or others (Baker Act or Marchman Act candidate).
  • Enforce program rules and expectations and reports infractions to the Therapist, Clinical Supervisor and Clinical Director.
  • Perform assessments of clients, within scope, and formulates treatment plans.
  • Monitor client conformance with program schedule including group attendance, reports problems and informs supervisor.
  • Maintain the confidentiality of all client and facility activities.
  • Makes daily assignment checks and discusses results with each client.
  • Comply with Policies and Procedures.
  • Facilitate weekly clinical groups.
  • Accept and utilize supervision.
  • Address problems noted by the supervisor.
  • Demonstrate willingness to accept responsibility.
  • Complete all tasks with concern for work quality.
  • Perform other assignments as requested by the supervisor.
  • Protect the privacy of all client information in accordance with GRNH, LLC’s privacy policies, procedures and practices, as required by federal and state law, and in accordance with general principle of professionalism as a healthcare provider. Failure to comply with the company’s policies and procedures on client privacy may result in disciplinary action up to and including termination of employment.
  • May access protected health information (PHI) and other patent information only to the extent that is necessary to complete your job duties. The incumbent may only share such information with those who have a need to know specific client information you have in your possession to complete their job responsibilities related to treatment, payment or other GRNH, LLC operations.
  • Expected to report, without the threat of retaliation, any concerns regarding GRNH, LLC’s policies and procedures on client privacy and any observed practices in violation of the policy to their supervisor.
  • Contract with payors upon request, ensuring all necessary documentation and requirements are met in a timely and accurate manner.
  • Maintain NPI and CAQH profiles, ensuring they are up-to-date and compliant with industry standards and regulatory guidelines.
  • Review, approve, and countersign all required clinical documentation in compliance with established timelines.
  • Holds daily individual sessions with each client under caseload.
  • Completes clinical documentation including bio psychosocial, initial clinical assessment and master treatment plan.
  • Provides aftercare planning to transitioning clients.
  • Follows up with family members and facilities family phone calls.
  • Monitors each client throughout the daily schedule.
  • Monitors client participation and reports problems to the supervisor.
  • Resolves conflicts through general and supportive advice.
  • Provides case management services.
  • Monitors and audits client ancillary accounts to ensure maintenance of necessary daily funds.
  • Facilitates the procurement of client daily living essentials.
  • Monitors and amends agency drug screening protocols/process as needed and under the supervision of direct supervisor.
  • Participates in client recreation, leisure, and weekend activities as scheduled/needed.
  • Maintains accurate and timely documentation regarding all client related activity.
  • Transports clients to scheduled activities or arranges transportation.
  • Report any vehicle problems to the Facilities Director.
  • Assist in collaborating with the client to develop a written, client-centered and recovery-oriented treatment plan with measurable goals and objectives, under the supervision of a licensed clinician.
  • Provide substance abuse counseling services specified in the client’s treatment plan, under direct supervision of a licensed clinician.
  • Participate in reviewing client progress throughout treatment using ASAM criteria and assist in determining the need for continued services or discharge/transfer, under the supervision of a licensed clinician.
  • Assist in developing client discharge or transfer plans to ensure appropriate movement between levels of care, with oversight from a licensed clinician.
  • Engage in case consultation activities with the clinical team, under supervision.
  • Document all counseling, education services, assessments, referrals, and follow-up in the client’s clinical record, with all entries reviewed and co-signed by a licensed clinician as required.
  • Uphold the organizational mission, values, and vision and support integrated care, demonstrating integrity and ethics in all activities.
  • Apply knowledge of substance use disorders, co-occurring disorders, and comorbidities in all supervised clinical activities.
  • Utilize knowledge of commonly used substances and their biopsychosocial impacts in supervised client care.
  • Participate in crisis intervention activities as part of the clinical team, always under supervision.
  • Co-facilitate individual, family, and group counseling sessions under the supervision of a licensed clinician.
  • Ability to recognize signs of withdrawal.
  • Client observation and monitoring skills/ procedures.
  • Knowledge of pregnancy related complications.
  • Knowledge of complications requiring transfers.
  • Knowledge of appropriate interventions.
  • Knowledge of frequently used medications including purpose, precautions, and side effects.
