About The Position

You're expected to drive for several hours each day, normally conducting 3-5 home visits (dependent on travel times).

Requirements

  • Bachelor’s degree required; preferably in social work, human services, or a related field.
  • LSW or LISW in state of employment preferred.
  • At least one (1) year of experience working with elders or adults with disabilities.
  • Ability to travel (by automobile) within assigned geographic area; valid driver’s license and automobile insurance is required.
  • Experience in case management, care planning and assessment for cases including but not limited to eldercare, people with disabilities, and other complex medical needs in a home care setting.
  • Training, evaluating, and supervising caregivers.
  • Using basic teaching principles, counsel and instruct the patient, family and caregiver in meeting their care related needs.
  • Proficiency in common software and web-based applications (e.g. MS Office, case management systems, PCP order forms etc).
  • Additional state-specific qualifications may be required.

Responsibilities

  • Conduct home visits to assess and create personalized care plans.
  • Train and support caregivers, ensuring they can meet their loved ones' needs.
  • Monitor and report on clients’ health, coordinating with their primary care physicians, community resources, and more.
  • Facilitate smooth transitions and ongoing care for clients.
  • Maintain compliance with all relevant regulations and standards.
  • Documenting in an accurate and timely manner; maintain a clinical record for each morbidity patients.
  • Monitoring the health status of all members to ensure that all needed care is properly delivered.
