The company is seeking a Care Manager for its team in Georgia, specifically for the Villa Rica, GA area. This is a field-based position requiring frequent travel to patient homes. Structured family caregiving involves a principal caregiver living with a patient to provide daily support, aiming to foster independence and offer evolving care options while preserving patient dignity. The Care Manager will be crucial to operations in Georgia, responsible for orientations, new patient assessments, and caregiver training.

Requirements

  • 2+ years of experience working with frail or elderly disabled adults and caregivers
  • Experience with data input, case review/case note entry
  • Working knowledge of ADL/ IADL assessments
  • Valid driver’s license with an insured vehicle for travel within the state
  • Excellent verbal and written communication, organizational and time management skills
  • Strong critical thinking and problem-solving skills
  • Proficient in Microsoft (i.e., Word, Excel, Outlook)
  • Excellent training skills
  • Team-player mentality

Nice To Haves

  • Previous field experience in a medical or case management environment
  • Salesforce experience
  • Hands-on experience in home care

Responsibilities

  • Performing orientations involving signing of paperwork
  • Conducting initial assessments during orientations
  • Performing periodic supervisory visits / check-ins for on-care cases
  • Training caregivers / patients as required per case
  • Interacting with the caregiver and other stakeholders, as needed
  • Engaging in interdisciplinary meetings with care managers, caregivers, and patients
  • Collaborating in care practice and communicating impressions of cases to appropriate staff
  • Critically assessing procedures and implementing improvements for evaluation of cases, feedback to appropriate parties, and training of caregivers
  • Wearing multiple hats as operations develop
  • Traveling within the state of Georgia (Atlanta Metro Area)
