About The Position

Provides support for care management and care coordination activities and collaborates with a multidisciplinary team to coordinate integrated delivery of member care across the continuum. The role strives to ensure members progress toward desired outcomes and contributes to the strategy of providing quality, cost-effective care. The Care Manager conducts behavioral health assessments, develops and monitors individualized care plans, maintains an ongoing member caseload, and coordinates behavioral health, long-term services and supports, and community resources. The position includes member outreach by telephone, face-to-face contact, or home visits as required, and may require an estimated 25–40% local travel depending on state and contractual requirements.

Requirements

At least 2 years of health care experience, preferably in behavioral health, or an equivalent combination of relevant education and experience.
A behavioral health clinician license or credential, such as LCSW, LMSW, APSW, CHES, LPC, LPCC, LMFT, PhD or PsyD in psychology, or an equivalent credential accepted under applicable state contract, regulation, or licensing mandate. Any license must be active and unrestricted in the state of practice.
Valid and unrestricted driver’s license, reliable transportation, and adequate auto insurance for job-related travel, unless otherwise required by law.
Experience working with people with severe and persistent mental health concerns and serious emotional disturbances, including substance use disorder and foster care.
Knowledge and experience related to whole person care principles, chronic health conditions, and discharge planning coordination.
Data entry skills and prior experience using a clinical platform.
Excellent verbal and written communication skills.
Proficiency with Microsoft Office suite or applicable software programs.

Certified Case Manager (CCM).
Experience in behavioral health care management.
Field-based care management or home health experience.

Responsibilities

Complete comprehensive behavioral health assessments within regulated timelines and determine eligibility for care coordination or case management based on clinical judgment, changes in health or psychosocial wellness, and assessment triggers.
Develop and implement care plans with members, caregivers, physicians, other appropriate healthcare professionals, and the member’s support network.
Conduct telephonic, face-to-face, or home visits as required.
Monitor care plans, evaluate effectiveness, document interventions and goal achievement, and recommend changes as appropriate.
Maintain an ongoing member caseload and conduct regular outreach and management.
Promote integration of behavioral health services, long-term services and supports, and home and community resources to enhance continuity of care.
Facilitate interdisciplinary care team meetings and informal collaboration.
Use motivational interviewing and Molina clinical guideposts to educate, support, and motivate members during contacts.
Assess barriers to care and provide coordination and assistance to address member concerns.
Provide consultation, resources, and recommendations to peers as needed.
