About The Position

We are seeking a highly motivated and compassionate Care Manager to join our team in the Social Assistance industry. Spanish-speakers are strongly encouraged to apply. As a Care Manager, you will be responsible for ensuring that our clients receive the highest quality care and support. Provide comprehensive care management services to clients, including assessment, planning, implementation, coordination, monitoring, and evaluation of their care plans. You will work closely with clients, families, and healthcare providers to ensure that all needs are met and that clients receive the best possible care. Care Managers are required to go into clients' homes and drive throughout Fairfield County. This position consists of 1 day in the office, 2 days in the field in the greater Bridgeport area, and 2 days working from home, once probationary period is completed.

Requirements

Bachelor's degree in Social Services, Human Services, or related field
Ability to drive throughout Fairfield County
Valid Driver's License
Strong communication and interpersonal skills
Knowledge of Medicaid and Long Term Care regulations
Patience and compassion
Skills in discharge planning and case management
Knowledge of community-based service practices

Responsibilities
