About The Position

FreedomCare is a healthcare company dedicated to revolutionizing the home care industry. We support patients by enabling them to choose a caregiver for in-home care. Our mission is to support patients across the U.S. We are looking for a Care Manager for our team in Georgia. This role requires fieldwork through the Fulton & Douglasville, GA area. Structured family caregiving is a caregiving arrangement where a patient lives with a principal caregiver who provides daily support based on the patient's needs. The goal is to provide necessary care while fostering patient independence in a home environment, offering a range of care options as needs evolve. This service provides alternative long-term care options for individuals who meet nursing facility-level care needs, while preserving the patient's dignity, self-respect, and privacy. The Care Manager will be a vital part of our operations in Georgia, performing orientations, assessing new patients, and training caregivers.

Requirements

2+ years of experience working with frail or elderly disabled adults and caregivers
Working knowledge of ADL/ IADL assessments
Valid driver’s license with an insured vehicle for travel within the state
Excellent verbal and written communication, organizational and time management skills
Strong critical thinking and problem-solving skills
Proficient in Microsoft (i.e., Word, Excel, Outlook)
Excellent training skills
Team-player mentality

Nice To Haves

Previous field experience in a medical or case management environment
Experience with data input, case review/case note entry
Salesforce experience is a plus
Hands-on experience in home care

Responsibilities

Performing home orientations involving signing of paperwork
Conducting initial assessments during orientations
Performing periodic supervisory visits / check-ins for on-care cases
Training caregivers / patients as required per case
Interacting with the caregiver and other stakeholders, as needed
Engaging in interdisciplinary meetings with care managers, caregivers, and patients
Collaborating in care practice and communicating impressions of cases to appropriate staff
Critically assessing procedures and implementing improvements for evaluation of cases, feedback to appropriate parties, and training of caregivers
Wearing multiple hats as operations develop
Traveling within the state of Georgia (Atlanta Metro Area)
