As a Care Manager, you will provide in-home support and companionship to individuals in our community while helping them live safely and comfortably in their own homes.

Requirements

A valid driver’s license
Reliable transportation
Current auto insurance
The ability to safely travel to multiple client locations as assigned
Caring and compassionate attitude
Dependable and professional work ethic
Strong communication skills
Ability to work independently
Must pass background checks as required

Nice To Haves

Experience in caregiving preferred, but willing to train the right candidate

Responsibilities

Assisting clients with Activities of Daily Living (ADLs), including: Bathing, Dressing, Grooming, Toileting, Mobility assistance
Meal preparation
Medication reminders
Light housekeeping and laundry
Providing companionship and emotional support
Assisting with errands and community activities
Maintaining accurate documentation and communication with the care team
Creating meaningful connections with clients and families through compassionate care
Travel between client homes throughout the shift.
