About The Position

The Care Manager RN role is within the Case Management Department at Navicent Health Medical Center. This is a full-time position requiring 40 hours per week, with a schedule typically from Monday to Friday, 8:30 am to 5:00 pm, and may include variable holiday and weekend shifts. The role involves managing patients and requires a current registered nursing license, with eligibility for a compact license if managing patients outside the applicable state. A minimum of three years of recent experience in acute care, home health, case management, discharge planning, or care management is required. Experience as a Care Manager and with IT solutions like electronic health records is preferred.

Requirements

  • Completion of an accredited nursing educational program
  • Current registered nursing license
  • Eligibility to manage patients in a facility outside of the state of applicable state requires a compact License
  • Minimum of three years of recent experience in acute care, home health, discharge planning or care management

  • BSN preferred
  • Experience as a Care Manager preferred
  • Experience with IT solutions such as electronic health record, learning management or disease/care management systems a plus
