About The Position

Our mission extends across the U.S., guided by core values of service, ownership, integrity, and a positive attitude. We are seeking a Care Manager, RN for our Connecticut team. This role involves conducting in-home assessments, patient and caregiver orientations, and providing ongoing care management and support. It requires a blend of clinical assessment, care coordination, patient/caregiver education, and quality assurance to ensure safe, effective, and person-centered care. The Care Manager, RN will collaborate with patients, caregivers, and interdisciplinary teams to develop care plans and facilitate service delivery. Regular home visits will be conducted to assess well-being, monitor care quality, and ensure regulatory compliance. Proactive communication of clinical concerns and changes in condition is essential for timely intervention and optimal patient outcomes.

Requirements

  • Diploma from an accredited nursing program.
  • Valid and active Registered Nurse (RN) license in the State of Colorado.
  • Ability to articulate cases through effective case summaries.
  • Ability and flexibility to travel (usually by car) within assigned geographic area; valid driver’s license and car insurance required.
  • Proficient in common software and web-based applications (e.g. Case Management Systems, MS Office) with the ability to teach others.
  • A passion for providing a high level of customer service, including the ability to show empathy, active listening, and patience.

Responsibilities

  • Conducts initial in-person clinical assessments and coordinates service activation, ensuring that all required forms and standard procedures are followed.
  • Develops, implements, manages patient's plan of care, making revisions and documenting changes as needed.
  • Actively coordinates the care of the client through communication and written documentation to the physician, and other disciplines involved in the client’s care and community resources.
  • Provide ongoing coaching to drive effective utilization of established best practices, encourage caregiver education, and connect families to appropriate resources.
  • Reviews the caregiver notes daily and appropriately addresses changes in the consumer’s condition or environment.
  • Ensure continuous compliance with all regulatory standards as well as company policies and procedures.
  • Performs other job-related duties as may be required or directed by the supervisor.
