About The Position

Care Manager RN at Petaluma Valley Hospital in Petaluma, CA. This position is Per Diem and will work 8 hour, Day shifts. Utilizing a collaborative process, the Case Manager will establish a continuum of care plan for a caseload of patients. The Case Manager will establish quality monitoring indicators, collection of data, analysis, documentation and the reporting outcomes, and liaison with patients. Requirements

• California Registered Nurse License
• 2 years of Acute care clinical or comparative experience.

Nice To Haves

• Bachelor's degree in nursing.
• CCM
• 3 years of experience in community health nursing or case management.
