About The Position

Riverside Healthcare is seeking a dedicated and compassionate Care Manager Registered Nurse to join our dynamic team. Requirements

  • Minimum of 3-5 years of experience in a clinical setting.
  • Current Registered Nurse (RN) license in the state of Illinois.
  • Strong clinical assessment and critical thinking skills.
  • Excellent communication, organizational, and interpersonal skills.
  • Proficiency with electronic medical records (EMR) and care management software.
  • Ability to work independently and as part of a team.
  • Strong commitment to patient-centered care and continuous professional development.
  • Speech: Needed for presentations, training, telephone communication, facilitating meetings.
  • Vision: Needed for computer entry, reading memos and literature.
  • Smell: Needed for electrical/fire safety.

Nice To Haves

  • Experience in managing complex cases and working with diverse patient populations is highly desirable.
  • Preferred previous experience with case management and utilization management.
  • New Graduate RNs starting on or after 1/1/25 must earn Bachelors of Science in Nursing (BSN) within 3 years of start date.
  • Bachelors Degree in Nursing (BSN) or higher from an accredited institution
  • Certification in Care Management (CCM) or similar certification is preferred.

Responsibilities

  • Develop, implement, and manage individualized care plans for patients with complex health needs.
  • Collaborate with multidisciplinary teams to ensure comprehensive and effective care.
  • Completes management functions including patient/family education.
  • Refers reviewed cases to a Physician Advisor as needed.
  • Conduct thorough assessments of patients medical, psychological, and social needs.
  • Create and adjust care plans based on ongoing evaluations and patient progress.
  • Serve as a primary contact for patients and their families.
  • Provide education on health management and resources, ensuring patients understand their care plans and treatment options.
  • Monitor and evaluate patient care and outcomes, coordinating with healthcare providers to address any issues or changes in patient condition.
  • Ensure timely follow-up and continuity of care.
  • Maintain accurate and up-to-date records of patient interactions, care plans, and outcomes.
  • Ensure compliance with regulatory requirements and organizational policies.
  • Identify and facilitate access to community resources and support services for patients.
  • Assist in managing care costs and optimizing the use of healthcare resources.
  • Participate in quality improvement initiatives to enhance patient care and optimize processes within the care management program.
  • Assist with administrative duties related to care management, such as preparing reports and attending meetings.
  • Contribute to the training and orientation of new staff members or interns as needed.
