About The Position

The RN Case Manager is responsible for carrying out effective case management functions for identified patients by planning, coordinating, negotiating, procuring and managing the care and needs of complex patients and their families throughout the continuum of care. The RN Case Manager assists patients in the utilization of appropriate health care services. Position may require Utilization Management and Discharge Planning, Chronic Disease Case Management based on inpatient settings. Ages of Patients Served: Infant/Toddler (0-3), Children (4-12), Adolescent (13-20), Adult (21-64), Geriatric (65+) Employee must demonstrate the knowledge and skills necessary to communicate with and provide care appropriate to the age of the patients served. Age-specific needs that the employee is required to know and understand will be reviewed and evaluated during job specific/department specific orientations and on-going thereafter. Requirements

  • Coursework/Training and recent completion of a post-graduate level curriculum focusing on case management Or equivalent education/experience
  • Current clinical experience with 2 years clinical experience in an acute care hospital setting, or 1 year current experience in an inpatient or outpatient case management setting.
  • Upon hire: California Registered Nurse License
  • 2 years of Case management, discharge planning, or utilization management experience in the acute, sub-acute, home health setting or managed care environment.

Nice To Haves

  • Bachelor's Degree in Nursing. Required to obtain within 3 years of hire date
  • 3 years of Acute Care Experience
