About The Position

The Patient Care Resource Manager (PCRM) is a nurse case manager that facilitates patients through the health care system, including pre-admission through post-discharge coordination of care, resources, and services utilizing the nursing process. The PCRM monitors and coordinates the patient plan of care to enhance the timeliness of patient care decisions and throughput, improve care quality and efficiency, and ensure efficient resource utilization. The PCRM identifies and removes barriers to the progression of the plan of care for each patient.

Requirements

  • Bachelor’s degree or equivalent combination of education and experience.
  • RN licensure required.
  • Five years recent clinical experience applicable to the population preferred.
  • Knowledge of clinical practice guidelines, computer data bases, and payer protocols desired.

  • Masters preferred.
