About The Position

You'll have the opportunity to build meaningful, long-term relationships with patients and families, help patients successfully manage chronic and complex health conditions, partner closely with physicians and interdisciplinary healthcare teams, improve care transitions between hospital, rehab, skilled nursing, home care, and community settings, support patients in overcoming barriers to care, drive better health outcomes and improve quality of life, and use critical thinking and problem-solving skills every day.

Requirements

  • Associate Degree in Nursing (ASN)
  • ASN with 10 years of acute care nursing experience; OR BSN with 5 years of acute care nursing experience
  • Active Registered Nurse license in Maryland or qualifying Nurse Licensure Compact (NLC) multi-state license
  • Current BLS/CPR certification
  • Ability to obtain active Pennsylvania and West Virginia RN licenses within 6 months of employment
  • Care/Case Management Certification required within 5 years of employment

Responsibilities

  • Manage a panel of high-risk, high-cost, and chronic condition patients
  • Assess patient needs and develop individualized, patient-centered care plans
  • Collaborate with primary care physicians and multidisciplinary teams to coordinate care
  • Coach and educate patients on disease management, preventive care, medications, and self-management strategies
  • Connect patients with community resources and support services
  • Promote engagement in healthy behaviors and self-care activities
  • Facilitate smooth transitions across the continuum of care, including acute care, rehabilitation, skilled nursing, home health, and outpatient settings
  • Monitor patient progress and adjust care plans to support optimal outcomes
  • Utilize evidence-based practices to improve quality, efficiency, and patient satisfaction
  • Advocate for patients, families, and caregivers throughout their healthcare journey
