Care Manager RN - Case Management
About The Position
The Registered Nurse Clinical Case Manager (RNCCM) provides care management and care coordination services for at-risk patients, including patients with complex medical and psychosocial issues. Population-based, chronic care management approaches and application of clinical protocols are integral to the program. This position is a core member of the patient centered medical home (PCMH) teams led by the physician/provider. Providence caregivers are not simply valued – they’re invaluable. Requirements
- Graduation from an accredited school of nursing.
- Oregon Registered Nurse License upon hire.
- 3 years clinical experience as an RN.
Nice To Haves
- Bachelor’s Degree in Nursing
- 2 years Home health, skilled nursing facility and/or community health nursing.
- 2 years Managed care experience and/or utilization management experience.
- 2 years Care/Case Management experience.
