The Care Manager Registered Nurse will be responsible for facilitating communication among patients, families, the multidisciplinary team, medical management team, and community resources. This role involves identifying and resolving barriers to care delivery, interpreting contracts to achieve optimal clinical and financial outcomes, and collaborating with various healthcare professionals to ensure quality, efficient, and cost-effective services. The position may involve working embedded within a provider office or telephonically. The Care Manager will utilize evidence-based approaches to enhance patient and family activation and engagement, partnering with patients to set SMART goals and assisting in the development of self-management educational resources. They will identify potential learning barriers and report abnormal findings to the responsible provider, collaborating on a plan. The role requires independent caseload management, ensuring timely completion of tasks and documentation for MCO, regulatory, and contractual requirements. The Care Manager will partner with at-risk patients throughout their care journey, anticipating needs, responding to changes in status, and prioritizing care based on essential patient needs. They will coordinate patient information and communication between all parties involved to ensure smooth transitions of care and coordinate referrals to internal and external resources as necessary. This role requires the ability to provide age-appropriate care, demonstrating knowledge of growth and development across the lifespan and assessing patient needs based on age-specific requirements.
