About The Position

The Care Manager, Telephonic Nurse 2, in a telephonic environment, assesses and evaluates members' needs and requirements to achieve and/or maintain an optimal wellness state. The Care Manager guides members/families toward and facilitates interaction with resources appropriate for the care and wellbeing of members. The Care Manager, Telephonic Nurse 2 work assignments are varied and frequently require interpretation and independent determination of the appropriate courses of action. The Care Manager, Telephonic Nurse 2 employs a variety of strategies, approaches and techniques to manage a member's physical, environmental and psycho-social health issues. Identify and resolve barriers that hinder care. Ensure patient is progressing towards desired outcomes by monitoring patient care through assessments and evaluations. May create member care plans. Understand department, segment, and organizational strategy and operating goals, including their linkages to related areas. Follow established guidelines/procedures. Use your skills to make an impact.

Requirements

  • 3 or more years of clinical acute care experience
  • Licensed Registered Nurse (RN) in the state of South Carolina with no disciplinary action
  • Comprehensive knowledge of Microsoft Office applications including Word, Excel, and Outlook
  • Minimum download speed of 25 Mbps and an upload speed of 10 Mbps for internet service
  • Work from a dedicated space lacking ongoing interruptions to protect member PHI / HIPAA information.
