The Care Manager Registered Nurse (RN) facilitates communication among patients/families, multidisciplinary teams, medical management teams, and community resources to identify and resolve barriers to care delivery. This role involves interpreting contracts to achieve optimal clinical and financial outcomes. The RN collaborates with patients, families, and inter-professional teams to ensure quality, efficient, and cost-effective healthcare services, potentially working embedded in a provider office or telephonically. They utilize evidence-based approaches to enhance patient and family activation and engagement, partnering to develop SMART goals and assisting with self-management educational resources. The RN identifies potential learning barriers and reports abnormal findings to the care team for plan development. They independently manage their caseload, ensuring timely completion of tasks and documentation for MCO, regulatory, and contractual requirements. The role involves partnering with at-risk patients throughout their care journey, anticipating needs, responding to status changes, and prioritizing care. The RN coordinates patient information and communication between various entities to ensure smooth transitions of care and coordinates referrals to internal departments and external community resources as needed. A key aspect is demonstrating knowledge and skills for age-appropriate patient care, understanding growth and development across the lifespan, and assessing patient status to meet age-specific needs.
