About The Position

HSHS St. John’s Hospital is seeking a proactive and compassionate Registered Nurse (RN) Case Manager to join our interdisciplinary care team. This role is essential to supporting patients throughout their hospital stay and transition of care by coordinating clinical services, assessing discharge needs, and ensuring optimal outcomes through timely and effective case planning. Requirements

  • Associate’s Degree or Diploma in Nursing is required.
  • 2 years of clinical experience is required.
  • License as a Registered Nurse (RN) in the state of practice is required.
  • Basic Life Support Certification (BLS) is required within 60 days of hire.

Nice To Haves

  • Bachelor’s Degree in Nursing is preferred.
  • Prior case management experience is preferred.
  • Quality/Process Improvement experience is preferred.
  • Case Management certification (ACM, CCM, RN-BC) is preferred.
