The Care Manager Registered Nurse GCM is responsible for facilitating communication among patients, families, the multidisciplinary team, and community resources to identify and resolve barriers to care. This role involves interpreting contracts to achieve optimal clinical and financial outcomes, and collaborating with various healthcare professionals to ensure quality, efficient, and cost-effective services. The position may be embedded in a provider office or conducted telephonically. The Care Manager utilizes evidence-based approaches to enhance patient and family engagement, develops SMART goals with patients, and assists in the development of self-management educational resources. They identify learning barriers, report abnormal findings to providers, and independently manage their caseload, ensuring timely completion of tasks and documentation for MCO, regulatory, and contractual requirements. The role involves partnering with at-risk patients throughout their care journey to ensure continuity, anticipating needs, responding to changes in patient status, and prioritizing care. Coordination of patient information and communication between various entities, including referring/accepting facilities, physicians, and community caregivers, is crucial for smooth transitions of care. Referrals to internal departments and external resources are also coordinated. The position requires the ability to provide age-appropriate care, demonstrating knowledge of growth and development across the lifespan and assessing patient status based on age-specific needs.
