Requirements

Master’s Degree of Science in Nursing
Current Nurse Practitioner licensure in Missouri (or eligibility)
Certified Nurse Practitioner (or eligibility)
Strong clinical, communication, and interpersonal skills
Commitment to patient-centered, team-based care
Ability to work independently

Nice To Haves

2 or more years experience as an Adult, Family or Gerontology Nurse Practitioner, is preferred.

Responsibilities

Focus on assessment, diagnosis, treatment, care coordination, and chronic disease management for patients in their homes or via telehealth platforms.
Provide service through core competencies of direct care, consultation, expert guidance, leadership, ethical decision making and collaboration.
Use your advanced clinical skills and training to assess, diagnose, manage, and treat patients.
Collaborate and participate in care coordination activities that meet the patients' needs.
Reduce barriers when managing discharge planning, directing transitions of care, and assisting patients throughout their health care journey.
Work collaboratively with physicians, care management teams, home health agencies, and other healthcare professionals to ensure high-quality, timely, and accessible care.
