Under supervision of Care Management Manager, works as a catalyst to promote the following: coordination of care, continuity of care, achievement of consistent and/or expected psychosocial outcomes, patient advocacy and efficient resource management. Requirements

• Master's Degree in Social Work or related field
• Washington Clinical Independent Social Worker Associate License Upon hire
• 3 years of Social services experience with a health care related organization
• minimum of 2 years in acute care/ care coordination

• Accredited Case Manager (ACM) Upon hire
• Certified Case Manager (CCM) Upon hire
