About The Position
Altais is a growing, dynamic team focused on transforming healthcare. As a Care Manager LVN, you will establish relationships and foster respect with Health Management Operations and Health Value Teams, working closely with Provider Partners to manage and coordinate hospitalized members who are at risk and vulnerable. The role requires a strong commitment to patient-centered care and promoting health, wellness, and an improved patient and provider experience. This is a field-based position in the San Francisco Bay area.
Requirements
- Current unrestricted California LVN licensure required
- Minimum of 3-5 years recent acute/hospital clinical experience.
- Computer skills proficient, experience in EMR, Microsoft Office Suite.
- Desired experience in managed care concepts and practice.
Responsibilities
- Conducting Initial and Concurrent Review and Discharge Planning/Care Coordination, utilizing evidence-based criteria coupled with clinical expertise and collaboration with Medical Directors to ensure appropriate admissions, level of care, and timely discharges.
- Monitoring and continually assessing and evaluating member severity of illness, intensity of services, and response to treatment, making recommendations for appropriate continued stay, transition to lower level/tertiary care, and coordinating member transitions.
- Applying clinical knowledge, critical thinking skills, solutions-oriented problem-solving, and strong decision-making approaches.
