About The Position
NADAP is seeking a Care Manager II to join their Health Homes Care Management team. This role is based in Patchogue, NY, with fieldwork across Suffolk County. The Care Manager will assist individuals with medical and behavioral health conditions by providing field-based support, including assessment, care planning, connections to healthcare providers, service coordination, client education, and linkages to community resources. NADAP is a non-profit agency focused on helping individuals with medical, behavioral health, and social service needs achieve self-sufficiency.
Requirements
- Bachelor’s Degree in human services, social work, or related field OR Associate’s degree with 1 year of case management experience OR High School Diploma with 2 years of case management experience
- Excellent organizational, communication, and computer skills
- Personal vehicle and valid driver's license required
Responsibilities
- Provide field-based support to individuals with medical and behavioral health conditions.
- Conduct assessments.
- Develop care plans.
- Facilitate connections to primary and specialty care providers.
- Coordinate services with treatment providers.
- Educate clients on diagnoses, medications, and treatments.
- Link clients to community-based resources and entitlements.
