About The Position

- The Care Manager I at LIFESTREAM BEHAVIORAL CENTER is responsible for coordinating and managing comprehensive care for clients, ensuring that they receive appropriate behavioral health services. This role involves working closely with clients, families, and healthcare professionals to facilitate effective treatment plans and improve client outcomes.

Requirements

  • Bachelor's degree in Social Work, Psychology, Nursing, or a related field.
  • Minimum of 2 years of experience in case management or care coordination within a behavioral health setting.
  • Experience working with diverse populations, including individuals with mental health and substance use disorders.
  • Proven track record of developing and implementing individualized care plans.
  • Strong understanding of behavioral health principles and practices.
  • Excellent communication skills, both verbal and written, to effectively interact with clients, families, and healthcare professionals.
  • Ability to assess client needs and develop appropriate care plans.
  • Proficiency in using electronic health records and other healthcare management software.
  • Strong organizational skills and attention to detail.
  • Ability to work independently and as part of a multidisciplinary team.
  • Knowledge of community resources and ability to connect clients with appropriate services.
  • Compassionate and empathetic approach to client care.

Responsibilities

  • Conduct initial assessments and ongoing evaluations of clients' behavioral health needs.
  • Develop individualized care plans in collaboration with clients, families, and multidisciplinary teams.
  • Coordinate with healthcare providers, social services, and community resources to ensure clients receive comprehensive care.
  • Monitor and document clients' progress, adjusting care plans as necessary to meet changing needs.
  • Advocate for clients' needs and rights, ensuring they have access to necessary services and support.
  • Maintain accurate and timely documentation of all client interactions and care management activities.
  • Educate clients and families about behavioral health conditions, treatment options, and available resources.
  • Participate in team meetings and case conferences to discuss client progress and collaborate on care strategies.
  • Stay informed about current best practices and developments in behavioral health care management.
