About The Position

Under the direction of the Director of Community Services, will be responsible for care management of at-risk older adults, disabled adults and adults eligible for Enhanced Care Management (‘ECM’) services for the BCHD Community Services Program. This includes coordination, improving health outcomes, enhancing satisfaction and reducing unnecessary healthcare utilization, focusing on the full range of client needs: medical, behavioral and social services.

Requirements

  • Bachelor's degree in a health or human services field (e.g., social services, gerontology, psychology, public health, counseling).
  • One to two years prior experience in Care Management, Clinical skills and/or volunteer supervision.
  • Experience includes conducting client assessments, assessing for mental health concerns and dementia risk, formulating care plans, coordinating programs and managing client cases specifically for the older adult population.
  • Familiarity with federal/state programs for elderly and disabled.
  • Knowledge of clinical theory, human development, systems theories, strengths-based perspectives, normal and abnormal psychology, group process and counseling modalities.
  • Knowledge of law as related to mandated reporting, consumer protection (California Board of Behavioral Science policies) and the Tarasoff Law.
  • Knowledge of ethically based practice and limits of scope of practice and BCHD’s guidelines, polices, and procedures.
  • Knowledge of end-of-life issues and continuum of care.
  • Excellent working knowledge of computers, including Microsoft Office programs (e.g., Word, Excel, Outlook, PowerPoint), and ability to learn software programs and databases.
  • Excellent professional-level verbal and written communication skills with an ability to represent the Beach Cities Health District in a professional manner at all times.
  • Excellent customer service skills.
  • Exceptional interpersonal skills and an ability to work with a broad range of people from a variety of backgrounds and experiences in both group facilitation and community outreach settings.
  • Ability to take initiative and be a collaborative team player.
  • Requires excellent organizational, project and program management skills.
  • Ability to work independently and remain flexible in a team environment; ability to perform a variety of tasks simultaneously and ability to maintain confidential documents and information.
  • First Aid and Cardiopulmonary Resuscitation (CPR) methods; appropriate safety precautions and procedures; simple record keeping procedures.
  • Must be able to successfully pass background check, tuberculosis test and drug screen.
  • Must have a valid Class C California driver’s license and ability to maintain insurability under the District’s Employee Use of Automobiles Policy.

  • Experience working in multi-cultural and community-based settings preferred.
  • Relationship building skills and partner management experience is highly desirable.
  • Bilingual Spanish-English skills are highly desirable.
  • This position requires local travel and the ability to navigate public transportation with clients.

Responsibilities

  • Conducts in-depth assessments and reassessments in the client's home and identifies functional limitations which impede independent living; conducts visual assessment of client and living environment.
  • Formulates personalized care plans and implements services; follows up and monitors care plan in collaboration with client’s care team: client, family as available, healthcare providers and community resources. Adjusts care plan as needed to meet client goals.
  • Serves as primary point of contact for clients and care team. Identifies and develops support systems for clients; provides support to clients and caregivers; provides information and referrals; conducts client advocacy as appropriate and manages a client caseload with guidance from their supervisor and/or other team members as appropriate. Serves in the role of Lead Care Manager under ECM including but not limited to: educating clients on ECM services; assisting with enrollment in ECM; establishing and maintaining trusting relationships and facilitating communication with clients and other members of care team and leading the client’s multi-disciplinary care team and participating in care coordination meetings.
  • Prepares monthly caseload reports and manages timely caseload documentation in the care management software in compliance with agency, state and federal regulations. Provides updates on status of cases during supervision with manager.
  • Provides counseling to care management clients and support group facilitation depending on community need and health priorities.
  • Provides support to volunteers and works with Volunteer Services Department to support client/volunteer relationship.
  • Establishes and maintains community linkages and serves on community-based committees and task forces.
  • Represents the District at health fairs and other community events and attends workshops, meetings and trainings as requested.
  • Provides training and supervision to assigned social work intern.
  • Maintains confidentiality; assures all participant records are maintained accurately in compliance with HIPPA standards.
  • Performs assigned training, duties and work schedule as a Disaster Service Worker on behalf of BCHD.
  • Exhibits clear understanding of the history, mission, and vision and serves as an ambassador of Beach Cities Health District.
  • Exemplifies BCHD’s core values of compassion, integrity, accountability, and excellence in every interaction with staff, volunteers and members and the public.
  • Performs special projects and other duties as assigned.
